For a long time, doctors believed that PANDAS—Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal infections—was caused by something called molecular mimicry. The idea was that after a strep infection, the immune system would get confused and start attacking the brain by mistake, leading to sudden-onset OCD, tics, anxiety, and mood swings (to name just a few common symptoms). This explanation made sense to many, but at the same time, it raised a lot of concerns—because if this was truly an “autoimmune attack,” what did that mean for long-term recovery?
But more recently, researchers have started questioning this theory. What if it’s not about mistaken identity at all? What if, instead, the immune system gets stuck in a state of overdrive, continuing to produce immune cells even after the bacteria are gone? Instead of just attacking the brain, this chronic inflammatory loop could be affecting the entire system, leading to persistent symptoms, flare-ups, and a nervous system that never fully settles.
